Please read our Adoption Guidelines page before filling out this form so you have an idea of what we are looking for.

We are pleased that you want to adopt you next horse from a rescue. This is our pre adoption application but it is NOT our final adoption contract. This is the first step in the adoption process, as explained further in our website. Please answer the following questions as completely & honestly as possible. When you are finished, simply submit the form online (quickest) or by mail (please allow appropriate time for a response).

If your desired horse is released to you for adoption you will be required to pay the stated adoption donation (depending upon the horse) that will include the cost for procurement price (if any), all necessary shots, worming, and farrier care. Adoption donations may be made by money order, cash, paypal, or cashiers check. Due to recent problems with adoption donations NO personal checks will be accepted.

Please Note: Adoption donations are NON-REFUNDABLE on horses, and the adopter understands that any horse coming back into LFSAR's care will not justify a refund. If a horse is returned to the program within 14 days after placement, the adopter will receive credit for the adoption donation paid, which may be put toward the adoption of another horse only in certain situations (No credit after this exchange period).

OWNERSHIP: Once adopted, a horse remains the property of LFSAR for the rest of its natural life! It may only be transferred back to said program. The horse may not be sold, given away, disposed of, or transferred to anyone but LFSAR nor may the adopter release the horse from his/her care.

CONDITION, TEMPERMENT, AND SOUNDNESS: Horses placed by LFSAR include no guarantees pertaining to general condition, temperament, or soundness.

Thank you for taking the time to fill out this pre-adoption application. We will contact you after we review it. Before you will be able to take your adoptive animal home you will be required to sign an adoption and placement contract and pay the stated adoption donation as well as submit to a farm check.

We require that the adopted equine be picked up within 30 days of the approval for adoption. Any equine that is unable to be picked up within that time period will be given a flat board fee of $5 per day until the horse is picked up. The board fee; in addition to any adoption donation still payable; will be due before the equine leaves the LFSAR farm.
